How to Respond to the No and Partial Scores in Questions 1 to 15
If you realized that there were gaps in your knowledge, skills, and judgment after your assessment, please describe how you have addressed these gaps or any plans you have made to address these gaps. Are you planning to take a course (if so, please list the course) or have you done any specific readings or taken any other actions for example? Provide as many examples as you like of actions you have or will take.
Refer back to the specific example discussed with the assessor during the assessment. The Committee appreciates when you describe your knowledge, skills and judgement, but it is even more helpful when you describe how you applied or how you will apply your knowledge, skills and judgement going forward.
Take this opportunity to explain and clarify any part of your practice that you feel the assessor may not have fully understood.
How to Respond to Concerns Identified in the Chart Review Section of the Assessment
Please look at the scores you received in the chart review section of the assessment.
You only need to provide a response to any section where you did not get three yeses (Yes), or you did not get three not applicables (NA).
- When responding, share any changes you have made to your record keeping practices since doing the assessment.
- When possible, provide record keeping samples (with no patient identifiers!) that show the improvements you have made to your record keeping. You can attach these files when you respond to the assessment report.
Questions to Ask Yourself When Writing Your Response to the Assessment Report
- How has my practice changed since doing the screening interview and the assessment?
Show the Quality Assurance Committee how you have made changes to your practice. The Committee likes to see examples of the changes you have made, so consider attaching documents or including an overview to show the changes you have made.
- What did I learn through the assessment process about my practice?
Describe any new knowledge or insights about your practice you gained from doing your assessment. What did the assessment make you think about or consider that you had not before?
- How is my past history with the College relevant or not relevant to my assessment results?
(Note: This does not apply to all physiotherapists)
When the Committee reviews your report, they will also consider any prior history that you have with the College. Your history could include education letters sent to you by the College, information received by the College, or a decision made the Executive Committee or Complaints Committee (when they reviewed cases) or decisions made by the Registration Committee, Inquiries, Complaints and Reports Committee, Discipline Committee, Fitness to Practise Committee or previous decisions made by the Quality Assurance Committee.
