How to Apply for a Provisional Practice Certificate of Registration

Below are all of the documents you will need to apply for Provisional Practice Certificate of Registration.

Read all of the information carefully and complete each required form. Include the Checklist when you submit your application.

As of April 2017, you do not require a Supervisor or employment site to apply for a Provisional Practice Certificate.
