If you have practiced as a physiotherapist or a member of another regulated profession in any other jurisdiction, you will need to arrange for the licensing board/college in the other jurisdiction to send the completed Regulatory History Form to the College of Physiotherapists of Ontario from each jurisdiction where you have been registered.
Important:
- The Regulatory History Form must be sent directly from the licencing authority to the College of Physiotherapists of Ontario
- The form must be dated within 6 months of applying to the College of Physiotherapists of Ontario.
- If the form received is incomplete, a revised form will be required.
Other Options
- A certificate of standing/letter of standing is acceptable only if the certificate/letter includes all of the same information as required on the College’s form. If the certificate/letter of standing does not contain the same information as the Regulatory History Form, the College will contact the licensing authority to obtain the additional information.
- If you have attempted to obtain a letter of professional standing/completed Regulatory History form and you have been unsuccessful and have waited more than six weeks, please send us the information that you have which would support that you have tried to obtain these documents. The College may allow for the completion of a notarized statutory declaration as an alternative.
Please note that we cannot approve your application for a certificate of registration until we have this information from all of the jurisdictions where you have practiced as a regulated professional.
