Independent Practice Application Checklists
New Registrants
For those applying for an Independent Practice Certificate of Registration for the FIRST TIME, please ensure that your application includes all of the following:
Application Form
Evidence of Canadian citizenship (i.e. valid Canadian passport, birth certificate or citizenship card), permanent resident status, or a valid work permit. A photocopy of this document is required.
Evidence of Name Change (i.e. a photocopy of your marriage certificate, divorce decree or official name change document) if applicable.
Evidence of a degree in physiotherapy. You can satisfy this requirement through one of the following:
- Submit photocopy of your degree; or
- Arrange for notification to be sent directly to the College of Physiotherapists of Ontario from the educational institution which issued the degree.
Evidence of successful completion of the Physiotherapy Competency Exam (PCE) (i.e. a copy of your PCE Certificate). If you have not completed the PCE, evidence of current registration with a regulatory body in a jurisdiction in Canada or evidence of registration with a regulatory body in a Canadian jurisdiction on December 30, 1993 is required.
Proof of registration/licensure and professional standing in all other jurisdictions where you have been registered/licensed as a physiotherapist. This can be done through a regulatory history form or letter of professional standing. This information should be dated within the last six months.
1,200 practice hours within the previous five years. This is only required if you have not completed the PCE in the previous five years and should be recorded on your application form.
The appropriate fees (non-refundable $114 application fee and pro-rated registration fee).
It takes 15 days to review and process a complete application.
Returning Registrants
If you are returning to the College after having held Independent Practice Registration in the past, you DO NOT need to provide any documentation that we required at your initial application for Independent Practice. You would be required to submit the following:
