  Provisional Practice Application Checklist

  Apply for a Provisional Practice Certificate of Registration if you have successfully completed the written portion of the Physiotherapy Competency Exam (PCE) and you are registered for the Ontario Clinical Exam.

  Note if you have previously taken the clinical component of the PCE or attempted the Ontario Clinical Exam and were unsuccessful, you do not meet the requirements for Provisional Practice.

  
  Please ensure your application includes the following:

   Application Form

   Evidence of Canadian citizenship (i.e. valid Canadian passport, birth certificate or citizenship card), permanent resident status, or a valid work permit. A photocopy of this document is required.

   Evidence of Name Change (i.e. a photocopy of your marriage certificate, divorce decree or official name change document) if applicable.

   Evidence of a degree in physiotherapy. You can satisfy this requirement through one of the following:
  o Submit a photocopy of your degree; or
  o Arrange for notification to be sent directly to the College of Physiotherapists of Ontario from the educational institution which issued the degree.

   Evidence of successful completion of the written component of the Physiotherapy Competency Exam (PCE). For example, a congratulatory letter or results page issued by the Canadian Alliance of Physiotherapy Regulators.

   Proof of registration/licensure and professional standing in all other jurisdictions where you have been registered/licensed as a physiotherapist. This can be done through a regulatory history form or letter of professional standing. This information should be dated within the last six months.

   The appropriate fees (non-refundable $114 application fee and $85 registration fee).
  
  It takes 15 days to review and process a complete application.
  
  Important: you must be registered for the Ontario Clinical Exam to receive a Provisional Practice Certificate from the College. 
  
  You must pay the full exam fee of $1,985 at the time of registration. You will not receive a Provisional Practice certificate if you are not registered for the exam.

• Independent Practice Applications

  The following applicants may enter the PT Portal to apply for an Independent Practice Certificate of Registration:

  • New and returning members

  • Those who currently hold a Provisional Practice Certificate of Registration in Ontario. 

  • Registered physiotherapists from another Canadian province or territory

  Independent Practice Application Checklists

  New Registrants

  For those applying for an Independent Practice Certificate of Registration for the FIRST TIME, please ensure that your application includes all of the following:

   Application Form

   Evidence of Canadian citizenship (i.e. valid Canadian passport, birth certificate or citizenship card), permanent resident status, or a valid work permit. A photocopy of this document is required.

   Evidence of Name Change (i.e. a photocopy of your marriage certificate, divorce decree or official name change document) if applicable.

   Evidence of a degree in physiotherapy. You can satisfy this requirement through one of the following:

  • Submit photocopy of your degree; or
  • Arrange for notification to be sent directly to the College of Physiotherapists of Ontario from the educational institution which issued the degree.

   1,200 practice hours within the previous five years. This is only required if you have not completed the PCE in the previous five years and should be recorded on your application form.

   The appropriate fees (non-refundable $114 application fee and pro-rated registration fee).
  
  It takes 15 days to review and process a complete application.

  Returning Registrants

  If you are returning to the College after having held Independent Practice Registration in the past, you DO NOT need to provide any documentation that we required at your initial application for Independent Practice. You would be required to submit the following:

   Application Form

   If you have not completed the PCE, evidence of current registration with a regulatory body in a jurisdiction in Canada or evidence of registration with a regulatory body in a Canadian jurisdiction on December 30, 1993 is required.

   Proof of registration/licensure and professional standing in all other jurisdictions where you have been registered/licensed as a physiotherapist since the date that you were first registered in Ontario. This can be done through a regulatory history form or letter of professional standing. This information should be dated within the last six months.

   1,200 practice hours within the previous five years. This is only required if you have not completed the PCE in the previous five years and should be recorded on your application form.

   The appropriate fees (non-refundable $114 application fee and pro-rated registration fee)

• Courtesy Registration Application

  Courtesy Registration Application

  Courtesy registration allows registered physiotherapists from other jurisdictions who are coming to Ontario to teach a course, or participate in a course, event or research project for less than 30 days to be registered in Ontario.

  To apply for a Courtesy Registration please ensure your application includes the following:

   Application form

   Evidence of Name Change (i.e. a photocopy of your marriage certificate, divorce decree or official name change document) (if applicable).

   Evidence of a degree in physiotherapy. You can satisfy this requirement through one of the following:
  o Submit photocopy of your degree; or
  o Arrange for notification to be sent directly to the College from the educational institution which issued the degree 

   Proof of registration/licensure and professional standing in all other jurisdictions where you have been registered/licensed as a physiotherapist. This can be done through a regulatory history form or letter of professional standing. This information should be dated within the last six months.

  If you are currently practicing in another Canadian jurisdiction, please note that we only require proof of registration/licensure from that jurisdiction.

• Independent Practice Certificate—Cross Border Physiotherapy

  Independent Practice Certificate—Cross Border Physiotherapy Checklist

  This checklist is for physiotherapists who currently practice in another Canadian province and who wish to provide care to patients in Ontario, whether through tele-rehabilitation or in person using an Independent Practice Certificate of Registration—Cross Border Physiotherapy. Please ensure your application includes the following:
  
   I have completed the application form.
  
   Non-refundable application fee of $113 and registration fee of $113.
  
   I have or will get professional liability insurance before I begin to provide patient care.
  
   I have previously or I am currently practicing as a physiotherapist in another province or territory and I have made arrangements with that regulatory body to send a Letter of Professional Standing directly to the College of Physiotherapists of Ontario.
  
   I understand that I am granted an Independent Practice Certificate of Registration—Cross Border Physiotherapy, I will only provide care to Ontario patients for the purpose of continuing to provide care for those whose physiotherapy began in another province and who would benefit from continued and time-limited service in Ontario or where services are not available in Ontario but would benefit patients.
